Home Health Transitional Care Nurse Practitioner (NP)

A Nurse Practitioner is needed in Rockford, Illinois.
Rockford is a welcoming city known for its riverfront parks, historic architecture, and easy access to outdoor recreation. This opportunity is with an innovative healthcare organization dedicated to improving the quality of life for older adults through a personalized, value-based home health model.

Salary and Benefits

• Base salary: $105,000 – $135,000 annually (based on experience)
• Annual clinical performance bonus:
  • Less than 5 years of experience: $15,000
  • 5+ years of experience: $25,000
• Total earning potential: $150,000 – $160,000 annually

• CME allowance and paid CME days (details provided upon offer)

Responsibilities

• Full-time, Monday–Friday schedule
• Focus on post-discharge transitional care visits in a home health setting
• Patient caseload: 6–7 patients per day
• Responsibilities include:
  • Medication reconciliation
  • Social service follow-ups
  • General follow-up care and care coordination
• Collaborate closely with a Medical Director for clinical guidance
• Deliver high-quality, patient-centered care designed to reduce hospital readmissions and promote wellness at home

Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program graduate
• Active and unrestricted Nurse Practitioner license in Illinois
• Active DEA license
• National board certification in one of the following:
  • Family Nurse Practitioner (FNP)
  • Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
  • Adult Nurse Practitioner (ANP)
  • Gerontological Nurse Practitioner (GNP)
• US work authorization required
