Primary Job Objective/Job Summary
The Community Health Worker will extend the reach of SC Thrive by serving as an essential bridge between the community and the organization’s partner sites. They will build strong relationships with partners and help clients navigate complex social services systems while using Thrive Hub to connect individuals and families to resources and programs. This role will become a subject matter expert on community resources and federal and state funded programs to effectively support partners and clients.

Working Conditions and Physical Demands:
Most of the time, the job will be performed in the community at partner and potential partner sites, events, rural locations, standard home office, and SC Thrive main office. Requires frequent travel, walking, setting up event space, and often works in outdoor weather conditions.
Must be able to work with individuals of various diverse backgrounds and socio-economic statuses. Consistently communicates information and ideas to others. Must be able to exchange accurate information in a manner others understand in these situations. Frequently meets with the public and observes details at close range (within a few feet of the observer). Consistently communicates information and ideas to others. Occasionally moves boxes up to 20 pounds for outreach events, materials, and for various purposes.
