Utilizing clinical social work techniques and theory, consults on, assesses, and intervenes with high-risk patients and their families regarding the impact/amelioration of emotional and social problems. Duties include conducting psychosocial assessments, making psychosocial diagnoses, planning and implementing effective treatments, evaluation, education, coaching and mentoring of patients and family members. The position consults and collaborates with social, governmental, legal agencies, courts, schools, clinics, hospitals, physicians, and other sources in patient care. The Pediatric Hematology Social Worker provides comprehensive psychosocial support to children with hematological conditions including Sickle Cell Disease and their families, working closely with the multidisciplinary hematology team. The role supports patients from diagnosis through adolescence and transition to adult care, in both outpatient and inpatient settings. Additional duties include cross-coverage in other specialty clinics such as OB/GYN, Neurology, and Oncology. Responsibilities include psychosocial support, biopsychosocial and trauma assessments, brief solution-focused interventions, trauma-informed crisis interventions, therapeutic relationship development, motivational interviewing, cognitive-behavioral treatment, communication with medical teams and community providers, screening for social determinants of health and protective needs, reporting abuse, providing referrals and advocacy, documentation in electronic health records, programmatic support including co-facilitation of support groups, organizing family support programs, compliance with documentation and confidentiality policies, participation in committees, and maintaining competency and professionalism. Requires a Master’s Degree in Social Work, Massachusetts LCSW license, 1-2 years of relevant experience, cultural sensitivity, computer literacy, and ability to work in a fast-paced environment.
